Multiple sclerosis (MS) has long been viewed primarily as an inflammatory disease driven by peripheral immune attacks on the central nervous system. However, emerging evidence suggests that disability can progress even in the absence of clinical relapses, a phenomenon known as progression independent of relapse activity (PIRA). This has led researchers to investigate chronic active, or “smoldering,” lesions and compartmentalized inflammation within the brain and spinal cord as key drivers of ongoing tissue damage. These smoldering lesions are characterized by a rim of activated microglia and macrophages that slowly erode myelin, contributing to neurodegeneration and progressive disability.
